How To Register
- Patients need an appointment to be seen. For established patients, it is often possible to get a same day appointment. Call 541-773-3863 to make an appointment.
Current Address and Phone Number
- It is very important that we have our patient's correct address and phone number in case we need to contact them to follow-up on a medical problem. Please be sure to let us know of any changes. This will also help us inform patients of important changes at the clinic, such as clinic hours, new services or special events.
